AD from a cop's gun kills a woman

I understand that the PD there uses a S&W M&P in .40S&W.
I'm not anti-Glock or anti-M&P, but it makes you wonder if this would have happened with a regular SA/DA gun.
We need more information before we make any judgement, but this is a terrible accident.

The first reports are saying IWB holster . . .
. . . pretty sure that one does not make any sense for a hit in the chest.
Could be a coverup for playing with the gun.

Sorry, I should have said these comments were made at the
shop yesterday.
It is a sad thing made worse by the surety of avoidance. It is such an odd thing that we can't imagine how it happened without a detailed description.

I was thinking "Miami Vice"...
A' la Don Johnson...horizontal, cross-draw but the AD still seems pretty tough. That was the only thing I could come up with that put the muzzle in the right place.
Regardless, I hate seeing stuff like this.
